Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was completed on February 16, 2021. The provider was contacted by telephone (video call) to initiate the inspection. There were 11 children present and in the care of five staff members. The inspector was able to view all required documentation. A virtual tour of the children in care was conducted. Five staff records and two children's records were reviewed. One medication was reviewed. Information gathered during the inspection determined a non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the facility.

The Licensing Inspector reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on the review of two children's records and verification from the director, the center failed to obtain all of the required enrollment information for one child.

Evidence:
1. The record for Child A did not contain a phone number for either of the two emergency contacts that the center is to call if there is an emergency and the child's parent can not be reached. The record did not contain an address for one of the emergency contacts.
2. The director confirmed that two phones numbers and one address had not been obtained for the child's emergency contacts.

Plan of Correction: During the inspection, the director was able to obtain the missing information and it was placed in the child's record. In the future, the staff will ensure that all enrollment information is complete.
