This inspection was conducted by staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the center was not conducted at this time however, a portion of the inspection included a virtual component which allowed the inspector to observe the facility.
A announced monitoring inspection was conducted at 10:00am. The program director was contacted by phone to initiate the inspection, and given a list of items required to complete the inspection. There were 25 children + 5 direct-care staff present. Ratios and supervision were in compliance. Classrooms were well supplied with toys and equipment for the children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliances with applicable standards or laws reviewed. Standard #: 22VAC40-185-550-M Description: Based on records reviewed, it was determined that 2 of 3 written injury reports did not include all required information. Evidence: 2 of 3 reports submitted for review did not include future action to prevent recurrence. Plan of Correction: We will review with teachers how to fill out injury forms completely
Standard #: 22VAC40-665-580-K Description: Based on records reviewed and interview conducted, it was determined that there was not at least one staff person on duty who has obtained Daily Health Observation training within the last 3 years. Evidence: Training documents submitted did not include this training, and the program director indicated there were no records of such training on-file. Plan of Correction: Ali + I both registered for the class and will complete it asap when they send links.
