This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 02/10/21 and concluded on 02/18/21. The licensee was contacted by telephone to initiate the inspection. There were 15 children present and 2 staff. The inspector emailed the licensee a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. A virtual tour and observation was conducted via video on 2/18/21. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-60-A Description: Based on record review and interview it was determined that the center did not ensure that each enrolled child had a record that contained all of the required information.
Evidence:
1. The record for Child #1 did not contain the enrollment form. There was no documentation of:
-parent's addresses and phone numbers
-parent's places of employment and phone numbers
-emergency contact names, addresses, and phone numbers
-person's authorized for pick up
-current and previous programs or schools attended
-first date of attendance
2. Staff #3 confirmed that this information could not be located.Plan of Correction: Center will ensure all children have the required information in their records.
