Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on February 23, 2021 and concluded on February 24, 2021. The director 35 was contacted by telephone to initiate the inspection. There were 35 children present and 10 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on documentation review and staff interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: Child #2 has a diagnosed food allergy (strawberries) and their was not a allergy action plan available for review. Child #4 had a diagnosed food allergy (peanuts, tree nuts) and the allergy action available for review was dated for the school year 2019-2020.

Plan of Correction: Center Director has spoken with parents of child #2 and child #4. Parent of child #4 has secured an updated 2020/2021 allergy plan which center director has put in the child's file. Parent of child #2 has scheduled an appointment with her child's doctor to secure an updated physical reflecting that he no longer has an allergy.
