Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on February 24, 2021. The in-charge person was contacted by telephone to initiate the inspection. There were 16 children present and 3 staff. The inspector emailed the person in charge a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Thank you for your cooperation today. Violations:

Standard #:	22VAC40-185-130-A
Description:	Based on record review and interview, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. Child #2's record (First date of attendance 02/02/2021) did not have documentation of immunizations. Staff #2 confirmed that Child #2's record did not contain immunization records.
Plan of Correction:	The center will get the immunization records from the family and put the document in the record.
Standard #:	22VAC40-185-60-A
Description:	Based on record review, children's records did not contain required information. Evidence: 1. Child #2's record (First date of enrollment-02/02/21) did not contain documentation of viewing proof of the child's identity and age.
Plan of Correction:	The center will view the birth certificate and document the child's record.
Standard #:	22VAC40-185-90--A
Description:	Based on record review, the center did not ensure that signed written agreements between the parent and the center were in each child's record by the first day of attendance. Evidence: 1. Child #2's record (First date of attendance 02/02/2021) did not contain parent agreements for authorization of emergency medical care, pick up ill child policy, and notification of communicable diseases policy.
Plan of Correction:	The center will provide the family with these agreements and have the family to sign the written agreement.

Description:	Based on observation and interview, the first aid kit did not contain all required items. Evidence: 1. The first aid kit did not contain the first aid instructional manual. Staff #2 confirmed that the first aid kit did not have an instruction manual.
Plan of Correction:	The center will purchase a first aid instructional manual.
