This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was completed on 2/22/21. The Program Director was contacted by email to initiate the inspection. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 5 children and 5 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-185-140-A Description: Based on file review, 4 out of 5 children's records do not have documentation of a physical exam within a month from their respective start dates.
1. Child #1 began care on 11/2/20 and does not have a physical on file.
2. Child #2 began care on 1/12/21 and does not have a physical on file.
3. Child #3 began care on 1/11/21 and does not have a physical on file.
4. Child #4 began care on 1/5/21 and does not have a physical on file.
Plan of Correction: We will ask the parents to bring this to the center.
Standard #: 22VAC40-185-70-A Description: Based on file review, 1 out of 5 staff files does not have written information to demonstrate that the individual possesses orientation training required by the job position.
1. Staff #1 did not have documentation of orientation training on file.
Plan of Correction: The Director believes staff #1 completed this, but was unable to locate the documentation during the inspection. He will complete another orientation form, if this is not located.
Standard #: 63.2(17)-1720.1-B-4 Description: Based on file review, 1 out of 5 staff records does not have results of a criminal history record from any state in which the employee has resided in the preceding five years.
1. Staff #3 does not have a criminal history record check for Maryland, a state in which they resided within the past 5 years.
Plan of Correction: This will be completed.
