Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was completed on February 23, 2021. The provider was contacted by telephone (video call) to initiate the inspection. There were 6 children present and in the care of two staff members. The inspector was able to view all required documentation. A virtual tour of the children in care was conducted. Two staff records and two children's records were reviewed. Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the facility. The Licensing Inspector reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-185-60-A
Description:	Based on the review of two children's records and confirmation from the owner, the center failed to obtain all of the required enrollment information for one child. Evidence: 1. The record for Child #1 did not contain an address for one person for the center to contact in case of an emergency and the parent cannot be contacted. 2. The owner verified that the address for the emergency contacted was not in the file.
Plan of Correction:	The owner obtained the address for the emergency contact immediately after the inspection. In the future she will ensure that all enrollment information is complete before the child begins attending the center.
Standard #:	22VAC40-185-240-D-5
Description:	Based on record review and discussion with the owner, there was not a staff member working at the center on 02/23/2021 who had obtained Daily Health Observation training. Evidence: 1. The records of Staff Member #1 and Staff Member #2 did not contain documentation that either staff has received Daily Health Observation training. 2. The owner confirmed that none of the five staff members had been trained in Daily Health Observation of Children.
Plan of Correction:	The one staff member who had current Daily Health Observation Training was no longer working at the center and the owner now understands that a staff member must always be on-site who has been trained within the last three years. She will ensure that enough staff members are trained so that someone is always present with the training.

Description:	Based on the review of two staff records and verification from the owner, the center failed to obtain an up-to-date National Criminal Background Check through fingerprinting prior to the first day of employment. Evidence: 1. The record for Staff Member #2 contained a completed National Criminal Background Check through fingerprinting dated 08/01/2020. The check was dated more than 90 days prior to the staff member's first day of employment. The date of employment for Staff Member #2 was 01/08/2021. 2. The owner stated that she was unaware that the background check dated more than 90 days before the hire date could not be accepted.
Plan of Correction:	The staff member has scheduled an appointment and will obtain an updated National Criminal Background Check through fingerprinting as soon as possible.
