This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 10/20/2020 and concluded on 01/19/2021. A
self-reported incident was received by the department regarding allegations in the
areas of forbidden action. The director was contacted by telephone to conduct the investigation. The inspector emailed the director/provider a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the self-report of
non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be
found on the violation notice.

Standard #: 22VAC40-185-400-B Description: Based on interviews and documentation, a facility staff failed to use behavioral guidance that was constructive in nature, age and stage appropriate, and intended to redirect child to appropriate behavior and resolve conflict. Evidence: Staff A grabbed a hold of child A to prevent him from running around and yelling during a fire drill. The child sustained a small scratch to his ear as a result of the incident. Plan of Correction: All staff were retained on appropriate behavior guidance. The staff member involved was placed on administrative leave immediately after the facility became aware of the situation and is no longer employed at the center.
