Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on May 18, 2020 and concluded on May 19, 2020. The provider was contacted by telephone to initiate the inspection. There were three children and two staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on a review of documentation, the vendor did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence: The center director confirmed that there allergies, sensitivities, and dietary restrictions had not been documented on the allergy list for Child #1 and Child #8.

Plan of Correction: The allergy list will be updated immediately in each classroom to show the allergies, sensitivities and dietary restrictions for each child.

Standard #: 22VAC40-665-520-B-11
Description: Based on a review of documentation, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The center director confirmed that there was not a an allergy care plan for Child #8 and the allergy care plan for Child #1 had not been signed by the physician.

Plan of Correction: Allergy care plans will be given to the parents to have the doctor to fill out the medical forms.
