Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 03/01/2021 and concluded on 03/03/2021. The provider was contacted by telephone to initiate the inspection. There were 13 children and 3 staff members present during the inspection. The inspector reviewed 2 children's records, 2 staff records and 3 medications during the inspection to ensure documentation was complete. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Description: Based on record review and discussion with the staff, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child #2 contained documentation of a confirmed food allergy. There was not a written allergy care plan for from a physician regarding the food to which Child #2 is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction

Plan of Correction: The provider will request a written allergy care plan from the parent.
