Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on February 26, 2021 and concluded on February 26, 2020. The director or in-charge person was contacted by telephone to initiate the inspection. There were ninety-nine children present and eighteen staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records and five staff records and one agent record submitted by the facility to ensure documentation was complete.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 63.2(17)-1720.1-B-4
Description: Based on review of five staff records, and staff interview, it was determined that no Out of State Child Abuse and Neglect Search follow up was conducted 45 days after the initial request to search.

Evidence:
1. The record for Staff #2, (date of hire: 08/19/2019) who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state search request or follow up.
2. The administrator confirmed that there was no documentation of out of state search request or follow up.

Plan of Correction: I had the teacher complete the paperwork again and resent the request for the out of state child abuse and neglect search. I will follow up in 45 days if I do not receive a response within 30 days.
