Technical Assistance:
1. Discussed cooling requirements when indoor air temperature reaches 80 degrees. Today the temperature was measured to be 79.2 degrees in the living/dining area.
2. Reviewed/trained vendor on new subsidy requirements.
3. Forms provided: proof of identity, new emergency plan, attendance form, and transportation permission.

Comments:
10:55am-12:15pm Annual unannounced subsidy inspection conducted. Observed 2 children = 2 points with the vendor. Ratios and supervision were in compliance. Children were playing games on a tablet and relaxing, as they did not feel well. The vendor and a visiting adult were talking with the children. The home was found to be very clean, and sufficiently supplied with toys for the children. The vendor is currently certified in First Aid/CPR. Recordkeeping and safety non-compliances found.

Note: Adult relative translated between inspector + vendor today.

Violations:
Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 1 of 2 children's records did not include required information. Evidence: Child #4 - Name, address and phone number for 1 of 2 parents.

Plan of Correction: The vendor will ask the parent to write "N/A" on the form.

Standard #: 22VAC40-665-210-B
Description: Based on records reviewed, it was determined that 1 of 2 children's records did not include immunizations updated every 6 months until the age of 2 years old. Evidence: Child #4 - Immunizations were dated June 2011, when the child was 1 year old.

Plan of Correction: The vendor will ask the parent for updated immunizations.

Standard #: 22VAC40-665-250-A
Description: Based on observations made, it was determined that potentially poisonous substances were accessible to children. Evidence: 2 cans of spray air freshener were on the window sill in the bathroom and within reach of the children.

Plan of Correction: The vendor put these items in a locked cabinet during the inspection.
