This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy monitoring inspection was initiated on December 31, 2020 and concluded on March 2, 2021. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the non-compliance with subsidy regulations, and violations were issued.

Standard #: 22VAC40-665-550-2 Description: Based on interviews and documentation review, the vendor did not inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence: 1) On 12/17/2020, Child #1 was injured at the center. The vendor was notified on 12/18/2020 that the child was seen by a physician on 12/17/2020. 2) The vendor submitted the serious injury report to the department on 12/23/2020.Plan of Correction: Not available online.
