This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 03/10/2021 and concluded on 03/11/2021. The director was contacted by telephone to initiate the inspection. There were 91 children present and 15 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 63.2(17)-1720.1-B-4 Description: Based on record review and interview, the center did not ensure that the results of a criminal history record information check and a sex offender registry were obtained from each state an employee had resided in within the past 5 years prior to the first date of employment as required.
Evidence:
1. The record of staff #1 (DOH: 02/15/2021) contained a sworn statement that indicated that the employee lived outside of the state of Virginia within the past 5 years.
2. The record of staff #1 did not contain documentation of an out of state criminal history check or a sex offender registry check.
3. The administrator acknowledged that the checks had not been completed and indicated that it would be completed that day.Plan of Correction: California does not perform these; notation and requests were made prior to completion of the inspection and submitted with required acknowledgement-Will keep and maintain records accordingly for all out of state checks in the future.
