Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 03/08/21 and concluded on 03/11/21. The director was contacted by telephone to initiate the inspection. There were 119 children present and20 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and (#) staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a record review the center did not ensure all children had the required immunizations.
Evidence:
Child #3, did not have an immunization record that was signed by the child's physician.

Plan of Correction: The following plan of correction was provided by the director during the exit interview.
The director will contact the child's parents to obtain a signature on the shot record by the child's doctor.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas of the center were maintained in a safe condition.
Evidence:
There was peeling paint along the door frames of the bathrooms.

Plan of Correction: The following plan of correction was provided by the director during the exit interview.
The entire building has been approved to be painted within the next 30-day days. In the meantime, the areas with peeling paint will be lightly sanded to help curb the paint from peeling until it is corrected.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center did not ensure electrical outlets had protective covers.
Evidence:
In the Pre-k Prep room, there was a plugged-in powestrip that did not have protective covers over the electrical outlets.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will cap all electrical outlets.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on a record review the center did not ensure all staff had the required background checks.
Staff # 5 , hired 01/27/20, indicated on the sworn statement as living outside of the state of Virginia within the last 5 years. Staff #5 did not have documentation of a criminal record of sex offender check from that state.

Plan of Correction: The following plan of correction was provided by the director during the exit interview.
The director will contact send for both checks this week. This was an oversight.
