This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on February 19, 2021 and concluded on February 23, 2021. A complaint was received by the department regarding allegations in the areas of staffing and supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-350-E-1 Complaint related: No Description: Based on interview, the facility did not ensure compliance with the staff to children ratio required for children from birth to the age of 16 months.
Evidence:
1. Staff #1 and Staff #2 stated the infant 1 classroom had been out out ratio during the week of November 16, 2020 with up to ten infants with two staff.
2. Staff #1 and staff #2 stated Infant 1 classroom and Infant 2 classroom were being combined on occasion in November and December of 2020 due to lack of staff placing the classroom out of ratio.
3. Staff #1 and staff #2 stated documentation of attendance sheets for the Infant class had been changed by a former staff to reflect staff and children were in ratio when they were not.Plan of Correction: Per the Director: I recently stepped in as the new director of this location. I have hired additional staff and increased communication with staff to ensure we meet the required ratios daily.
