Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on 3/15/2021. The director was contacted by telephone to initiate the inspection. There were (28) children present and (8) staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed (3) children?s records and (3) staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Violations:

Standard #:	22VAC40-185-60-A
Description:	Based on record review, children's records did not all contain required information. Evidence: 1. Child #2 was missing two emergency contact addresses. 2. Child #3 was missing both parent's place of employment and one parent's phone number and the 2nd emergency contact address and phone number.
Plan of Correction:	We will ask the parents for the information.
Standard #:	22VAC40-185-70-A
Description:	Based on record review, required records were not kept for each staff person. Evidence: 1. Staff #2 had documentation that references were called on 9/12/11 with a time and initial, but it does not include the results or have the signature of the person that made the call.
Plan of Correction:	We will resend for the references.
Standard #:	22VAC40-185-90--A
Description:	Based on record review, the center did not have the required written agreement signed between the parent and the center for each child's record by the first day of the child's attendance. Evidence: 1. Child #2 (start date 9/14/2020) and Child #3 (start date 9/14/2020) did not have the required signed agreement between the parent and center by the first day of attendance.
Plan of Correction:	I will get these agreements signed.
Standard #:	22VAC40-185-260-B
Description:	Based on interview, the center did not have an annual approval from the health department. Evidence: 1. The last health department inspection was dated 10/17/2019 and Staff A stated they had not been out to do an inspection this past year.
Plan of Correction:	I will call the health department to schedule an inspection.
Standard #:	63.2(17)-1720.1-B-4
Description:	Based on record review, the center did not obtain a copy of the results of a sex offender registry check for any state in which the individual has resided in the preceding five years. Evidence: 1. Staff #1 lived in Florida in the last 5 years and the center did not have the results of the sex offender registry before 12/31/2020.
