This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on October 9, 2020 and concluded on October 21, 2020. A complaint was received by the department regarding allegations in the areas of operational responsibilities, procedures for emergencies, nutrition and food services and behavior guidance. The director was contacted by telephone to conduct the investigation. The inspector emailed the director/provider a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported non- compliance only a portion of the allegations related to procedures for emergencies. Non-compliance with standards or law and violations were issued. Any violations not related to the complaint(s) but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-550-L Complaint related: Yes Description: Based on review of documentation and interview, the facility did notify the parent by the end of the day of any known minor injuries.
Evidence:
The incident report indicates that the injury, a fall from a chair where child #1 hit her forehead on the floor, occurred on September 22, 2020 and the parent was notified and signed the incident report on September 23, 2020.Plan of Correction: Per the owner: We will start emailing parents a copy of the incident report the day of an injury to ensure parents are notified on that day. If a parent declines to sign/ acknowledge the injury report we will have two witnesses note on the the injury report the parents choice to decline.
Standard #: 22VAC40-185-550-M Complaint related: No Description: Based on review of four incident/accident reports, the facility did not maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and included all required documentation.
Evidence:
1. Incident/accident reports for child #1, dated 03/06/2020 and 07/15/2020 did not contain the actual time the parent was notified of the injury or how the parent was notified. Both incident/accident reports noted "at pick up" as the time.
2, Incident/accident reports for child #2, dated 06/24/2020 and 07/24/2020 did not contain the time the parent was notified of the injury or how the parent was notified. Both incident/accident reports noted "at pick up" as the time.Plan of Correction: Per the administration: Incident/accident reports will be completed in their entirety. We will review procedures with staff and ensure the exact time is listed. We will add to the form the method of contacting the parent as well.
