This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection with the local agency was initiated on 2/19/21 and concluded on 3/17/21. A self-reported incident was received by the department on 2/16/21 regarding allegations the care of the children. The director was contacted by telephone to conduct the investigation. The inspector did a virtual tour of the center and conducted interviews of staff. The evidence gathered during the investigation resulted in violations of the standards that are listed on the violation report.

Standard #: 22VAC40-185-340-A Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
1. On 2/16/21,while in care child #1 was given Lactose Free Milk by staff #1.
2. According to the child #1's record and the center's food allergy and dietary restrictions list, child #1 should not have been given this Lactose Free Milk .
3. Staff #2 confirmed that child #1 was given the Lactose Free milk in error.Plan of Correction: The facility responded: In an effort to correct the violation listed Children with a food allergy / preference will have the substituted item listed on our allergy list. We have implemented a mandatory training for all staff, ?Food Allergies and How to Manage them in Child Care Settings: Year 2). In addition to this all children in our care who have a listed allergy/ preference are records are now filed in in a red file folder instead of the standard purple / green. Our allergy lists that are posted in each classroom, office and kitchen is also printed on red paper in an effort to be more visible to all staff. The kitchen manager now delivers all milk alternatives in a cup, with the child?s name, the date, and the product name clearly labeled and visible on the outside of the cup. The director has taken additional online training courses ?Coaching and Mentoring: Supporting Staff?, as well as ?There?s Always More to Learn.?, in an effort to keep all staff abreast of all changes in a timely manner. Parent orientations are now conducted by the owner to assure that all parents are aware of our peanut / tree nut free policies, and the disenrollment policy for families that chose not to abide.
