Standard #: 22VAC40-185-340-F Description: Based on interview, a child under 10 years of age was not always within actual sight and sound supervision of staff.
Evidence -
1. On the date of inspection (03/23/2021), the center self-reported to the Division of Licensing that Child #1 (3 years of age) was left unsupervised for a total of 2 minutes on 03/19/2021 at approximately 4 pm.
2. The Director stated (during an interview conducted on the date of inspection) that Staff #1 failed to conduct a name to face roll call as the children in the Piglet Room transitioned from their classroom to the playground; leaving Child #1 alone in the classroom.
3. The Director found Child #1 (sitting on the room's carpet) while conducting a new staff tour and concluded that 2 minutes had past since Staff #1 left with the other children to the playground.
4. Including Child #1, there were a total of 5 children in the Piglet Room prior to Staff #1 taking the class to the playground at 4 pm.Plan of Correction: Staff #1 was terminated from employment. The Parents of Child #1 were notified of this incident involving their child.
