This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A self-reported incident was received by our office on 3/10/2021. An inspection was initiated on 03/10/2021 and concluded on 03/25/2021. A self-reported incident was received by the department regarding allegations in the areas of 22VAC40-185-240.A/B, 22VAC40-185-340.A/F/H. The director was contacted by telephone to conduct the investigation. A virtual inspection was conducted and the director provided a list of documentation to complete the investigation.
The evidence gathered during the investigation did not provide a preponderance of evidence to support the self-report of non-compliance with standards or law.
