Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 3/18/2021 and concluded on 3/22/2021. The director was contacted by telephone to initiate the inspection. There were 51 children present and 11 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

The provider?s response for the plan of correction was not received as of March 29, 2021 and will not appear on the Violation Notice.

Description: Based on a review of documentation, the center did maintain a current written list of all children's allergies, sensitivities, and dietary restrictions. Evidence: 1) The licensing inspector observed the record for Child #2 (start date 2/10/2020) contained documentation dated 1/28/2021; from the child's parent and from the child's physician, indicating an allergy to milk. 2) The licensing inspector observed that the center's allergy list, dated 3/1/2021, did not include the allergy information for Child #2 and this was confirmed by the director.
