Technical Assistance:
Training due 3/2020 Emergency planning and supplies proof of identity

Comments:
An annual SHSI inspection was completed today from 3:10 to 400 pm. There were 3 children ( 9 points ) in care at the time of the inspection. The inspector observed nap time, parent pick up and preparing for the arrival of kids in the afternoon. Household member, child information and emergency planning were reviewed as well as cleaning and self care routines. The home is well supplied with age appropriate activities and space for children in care. If there are any questions regarding this inspection please contact kelly.savittiere@dss.virginia.gov

Standard #: 22VAC40-665-170-B
Description: Based upon information reviewed, required information was incomplete. Evidence: Child A was missing parents school/ work address. B was missing emergency contact name/ address/ telephone number.

Plan of Correction: Mom came during inspection to fill out new form.

Standard #: 22VAC40-665-380-H
Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children. Evidence: Household medications were in a locked hutch in the dining area, however, the glass was missing on a door enabling access.

Plan of Correction: removed during inspection.
