This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A self-report was received in the Valley Licensing Office concerning an incident that occurred on 03/23/2021. An inspection was initiated on 03/25/2021 and concluded on 03/26/2021. The center was contacted by telephone to initiate the investigation. Five staff members were interviewed, two staff members' records were reviewed and one child's record was reviewed. On 03/26/2021 there were 54 children present and in the care of 17 staff members. Information gathered during the inspection determined a non-compliance with applicable standards and the violation was documented on the violation notice.

Standard #: 22VAC40-185-340-F Description: Based on the interviews with five staff members and verification from one staff member, the center failed to keep one child within sight and sound supervision at all times.
Evidence:
1. On 03/23/2021, Child A (age 20 months) was not within sight and sound supervision of a staff member for approximately 15 seconds when Staff Member #5 closed the door to the playground without realizing the child was still outside on the playground. Staff Member #5 then walked about 8 steps to the entrance to the classroom when Staff Member #2 asked the whereabout of Child A. Staff Member #5 immediately went back to the playground and found Child A.
2. Staff Member #5 reported that she was the last staff member to enter the building from the playground and walked to the entrance of the classroom when Staff member #2 informed her that Child A was not there. She immediately turned around, opened the door to the playground and found Child A standing on the playground.Plan of Correction: I have updated our procedures on supervision/tracking of transitions of children and all staff will be retrained on these procedures at our PDD on Monday 3/29/21.
