Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was conducted on 04/06/2021. A complaint was received by the department on 03/29/2021 regarding allegations in the areas Physical Plant and Special Services. There were 46 children present and in the care of seven staff members. The Assistant Director was contacted by telephone to initiate the virtual investigation. Four staff members were interviewed virtually and six classrooms were virtually observed.

The evidence gathered during the investigation supported the allegation and the complaint was determined to be valid. Two violations were issued.

Violations:
Standard #: 22VAC40-185-280-G
Complaint related: No
Description: Based on virtual observation of six classrooms and verification from staff, one classroom contained a bottle filled with a hazardous substance that was not labeled to indicate the contents.

Evidence:
1. A spray bottle in Classroom C containing a commercial sanitizing solution was not labeled to indicate what was in the bottle.
2. Staff Member #1 verified that the spray bottle was not labeled with the contents.

Plan of Correction: All bottles were correctly labeled and filled to their entirety for today. A memo was sent out to all staff
about correctly labeled sanitizing bottles and filled every morning and emptied every evening. We also
plan to have a staff meeting next Wednesday 4/14/2021, to reiterate the policy with sanitizing and
disinfecting and at all appropriate times, followed by our new procedure of emptying bottles in the
evenings and the openers refilling and passing them out in the morning.

Description: Based on virtual observation of classrooms, interviews and confirmation from a staff member, one staff member failed to sanitize the tables used for eating before and after a meal.

Evidence:
1. In Classroom A, Staff Member #4 did not sanitize the eating tables before and after breakfast on 04/06/2021 when she realized the spray bottle used for sanitizing was empty.
2. Staff Member #4 reported that when she arrived in the classroom on 04/06/2021, the spray bottle was empty and she did not tell anyone. She confirmed she had not sanitized the tables before or after eating.
