63.2(17) License & Registration Procedures
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An initial inspection was initiated on April 12, 2020 and concluded on April 16, 2021. The owner was contacted by telephone to initiate the inspection. There were no children present and 3 staff present during inspection. The inspector emailed the owner a list of items required to complete the inspection. The Inspector reviewed 4 applicant records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Standard #: 22VAC40-185-550-A Description: Based on documentation review and staff interviews, the licnesee did not ensure the center had an emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and shelter-in-place. The plan, which shall be developed in consultation with local or state authorities, addresses the most likely to occur emergency scenario or scenarios, including but not limited to natural disaster, chemical spills, intruder, and terrorism specific to the locality.
Evidence; The following items were not included in the center's emergency preparedness plan.
1) Establishment of center emergency officer and backup officer to include 24-hour contact telephone number for each
2) Staff training requirement, drill frequency, and plan review and update
3) Primary and secondary means of egress for evacuation
Plan of Correction: The center owner has updated the emergency plan.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review and staff interviews, the license did not ensure that all staff submit to fingerprint check prior to hiring.
Evidence: Staff #2 did not have documentation of obtaining a finger print check prior to hiring. Date of hire 4-15-21.
Plan of Correction: Staff #2 went to get background check today and waiting on results.
