This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 4/07/2021 and concluded on 4/13/2021. The director was contacted by email to initiate the inspection. There were 24 children present and eight staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Standard #: 22VAC40-665-540-A Description: Based on record review and discussion with staff, the center failed to ensure staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
1. The documented hire date for Staff #1 was 3/16/2021 and the documentation of his/her TB screening was dated 3/21/2021.
2. The documented hire date for Staff #2 was 7/1/2019 and the documentation of his/her TB screening was dated 7/15/2019.
