Technical Assistance:

An out of state search of the sex offender registry must be completed for all staff who have resided outside of the state of Virginia within the past 5 years. In addition, an out of state criminal history record search must be completed for staff who have resided in any non-NFF states within the past 5 years. This requirement also applies to staff who currently reside outside of the state of Virginia. If the out of state Criminal History record search is required, the results must be received prior to the staff person's first day of employment.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 4/13/2021 and concluded on 4/14/2021. The director was contacted by telephone to initiate the inspection. There were 36 children present and 7 staff. The inspector reviewed with the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-60-A
Description:	Based on a review of children's records, it was determined that two records do not contain all required information. Evidence: Child record A does not include the home phone number; the father's work phone number. Child record B does not include the home phone number; each parent's work phone number.
Plan of Correction:	Both parents will be asked for the information.
Standard #:	22VAC40-185-510-A
Description:	Based on a review of children's medications, it was determined that one medication was on site without all required authorization documentation. Evidence: Child C has one long term non-prescription medication on site. The parents have not completed an authorization form in order for this medication to remain on site.
Plan of Correction:	I will ask the parent to complete the authorization form.
Standard #:	63.2(17)-1720.1-B-2
Description:	Based on a review of new staff records, it was determined that two staff were permitted to begin employment without documentation to show that each person had submitted to the completion of fingerprint background searches. Evidence: 1 - Staff A began employment on 3/15/2021. There is no documentation showing that this person's fingerprint background search results have been received. 2 - Staff C began employment on 4/12/2021. There is no documentation showing that this person's fingerprint background search results have been received.
Plan of Correction:	They both got fingerprinted but the results were not received. I will make sure they both get fingerprint searches done again, and that information will be sent to the Inspector within the required 10 days.
Standard #:	63.2(17)-1720.1-B-4
Description:	Based on the review of new staff records, it was determined that management staff did not complete all required out of state background searches for two staff. Evidence: 1 - Staff A has indicated that she resided outside of the state of Virginia within the past 5 years. This person's out of state Sex Offender search, out of state Central Registry search, and out of state Criminal History search have not been completed. This staff person began employment on 3/15/2021. 2 - Staff B has indicated that she resided outside of the state of Virginia within the past 5 years. This person's out of state Sex Offender search and out of state Central Registry search have not been completed. This staff person began employment on 4/5/2021.
Plan of Correction:	The Sex Offender searches were completed yesterday 4/13/2021. The remaining out of state background search requests were submitted today 4/14/2021. The documentation of each request was forwarded to the Inspector today.
