Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated on 04/16/2021 and concluded in 04/19/2021 in response to a self-report incident that occurred on 04/15/2021. Interviews were conducted with staff, documents were reviewed and staff/child records were reviewed.

Areas of non-compliance not related to the self-reported incident were identified during the inspection and are listed in the Violation Notice. If you have any questions, please contact the Licensing Inspector, Maria Robles at maria.robles-lopez@dss.virginia.gov.

Violations:
Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, it was determined that the center did not ensure that Central Registry search results are obtained every five years for employees.
Evidence:
1) The most recent Central Registry search request results available in the record for Staff #1 were dated 04/04/2016. The director confirmed this is the most recent Central Registry search results in Staff #1's record.

Plan of Correction: A new Central Registry search was requested on this date for Staff #1.
