Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 15, 2021 and concluded on April 15, 2021. The administrator was contacted by telephone to initiate the inspection. There were twenty-five children present and five to six staff. The inspector emailed the administrator a list of items required to complete the inspection. The inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable licensing standards and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of records for children, and an interview with administrative staff, all required information was not contained in each record. Evidence: During a review of three records, the first date of attendance was not in the records. The record for child 3 only had the start date for a separate center.

Plan of Correction: The administrator immediately wrote the start date at the Luray center for child 3. The start dates will be determined for the other two children and written in the records. The administrator will review all children's records to make sure they include start dates in Luray.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on a review of staff records, and an interview with administrative staff, the center failed to obtain all required background checks required before employment for staff who resided in a state outside of Virginia during the the previous five years from the date of employment. Evidence: According to written information in a sworn statement in the file of staff member 2, one other state was listed as resided in within the past five years. There was not any documentation of out-of-state requests and results of a sex offender registry and a criminal history record name search. The administrator confirmed that only the out-of-state child abuse registry was obtained.

Plan of Correction: The administrator will follow the directions in the memo from June 2020 for requesting and obtaining out-of-state sex offender registry findings and the criminal record name search for staff member 2. The administrator will send copies of the requests to the licensing inspector. As the two background checks are obtained, they will be viewed, copies will be shared with the licensing inspector and maintained in the staff file. These types of background checks will always be obtained prior to new staff beginning employment.
