63.2(17) License & Registration Procedures

Technical Assistance:
All child care programs must continue measures to keep children physically distanced and with consistent caregivers, use face coverings, monitor children and staff for symptoms, and maintain ongoing compliance with the Virginia Department of Health (VDH) and Centers for Disease Control and Prevention (CDC) health and safety recommendations. All programs are required to notify VDH and the licensing inspector and follow all recommendations if there is a confirmation of a positive case of COVID-19.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was conducted on 4/21/21. The director was contacted by telephone and a virtual inspection was conducted. There were 68 children present with ten staff supervising. The Inspector reviewed four children's records, four staff records, the emergency drill log, medication, meals and snacks, enhanced cleaning and sanitizing of toys and surfaces, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of four children's records, the center did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: the record for child #1 (date of first attendance: 2/16/21 did not have documentation of immunizations.

Plan of Correction: Will ask the parent to provide documentation of immunizations.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of four staff records, it was determined that the facility did not have a repeat central registry report for all staff every five years as required. Evidence: The record for staff #1 had a central registry search report dated: 9/10/2015.

Plan of Correction: Will send off request for a new search immediately.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on review of four staff records, and staff interview, it was determined that not all staff had an Out of State Sex Offender Registry search or Out of State Criminal History Name Check, prior to their first day of employment. Evidence: the record for staff #2 (date of hire: 2/25/21) did not have search results from these out of state background checks.

Plan of Correction: Will follow up to obtain these out of state background checks, and ensure that future staff who have lived outside of Virginia in the past five years have these checks before employment.
