This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 04/15/2021 and completed on 04/20/2021. The program director was contacted by telephone to initiate the inspection. There were 25 children present with 4 staff during a nap time inspection. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations and a violation was documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-540-A Description: Based on record review and interview, the vendor failed to ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

a. Administrative staff verified that a TB screening had not been obtained for staff 1.Plan of Correction: Staff 1 will obtain a TB screening. Going forward all staff will obtain a TB screening prior to employment.
