Technical Assistance:
1. Reviewed appropriate storage for cleaning supplies to be out of reach of children.
2. Discussed safe sleep practices, and crib safety requirements.
3. Reviewed/trained vendor on new subsidy regulations.

Comments:
10:15am-12:20pm Annual unannounced subsidy inspection conducted. Observed 2 children = 5 points with the vendor. Ratios and supervision were in compliance. Children were observed playing with toys, washing hands, and having diaper changed during the inspection. The vendor was talking and playing with the children. The daycare areas of the home were found to be very clean, and well supplied with toys and equipment for the children. The vendor is currently certified in First Aid/CPR. Non-compliances in crib safety and safe sleep practices found, and the vendor has 10-days to make the needed corrective actions.

Standard #: 22VAC4-665-340-H-1
Description: Based on observations made, it was determined that a full-size crib used by a child enrolled does not meet the current federal safety guidelines, as it was not manufactured after 5/1/13. Evidence: According to the manufacturers label on the white wood crib was made in November 2003. This crib is used by Child #13.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC4-665-340-I
Description: Based on observations made and interview conducted, it was determined that a child under the age of 2 years old has been sleeping with a pillow. Evidence: A full-size bed pillow was observed in the crib used by Child #13. The vendor said the child will not sleep without the pillow.

Plan of Correction: Children will not be allowed to sleep with pillows until they turn 2 years old, or they have written permission from a medical doctor.

Standard #: 22VAC4-665-340-M
Description: Based on observations made, it was determined that mobiles were hanging over a crib used by a child that is over 5 months of age. Evidence: Two mobiles were attached to the side of the crib and hanging over the crib used by Child #13, who is 20 months old.

Plan of Correction: The vendor will remove the mobiles.

Standard #: 22VAC40-665-160-B
Description: Based on records reviewed, it was determined that a written hard copy record of daily attendance that documents the arrival and departure of each child as it occurs has not been maintained. Evidence: There was no written attendance records for 9/17/19 and 9/18/19.

Plan of Correction: The vendor will write attendance as child arrive and leave everyday.

Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 2 of 5 children's records did not include required information.

Evidence:

1. Child #13 - Parent home/work address and phone numbers; name, address and phone number for an emergency contact person; names of persons that may pick-up the child; and the sick-child policy.

2. Child #14 - Address for the emergency contact person; and the sick-child policy.

Plan of Correction: The vendor said that she is working with the parents to get the paperwork completed.

Standard #: 22VAC40-665-210-B
Description: Based on records reviewed, it was determined that 1 of 4 children's records did not include immunizations updated every 6 months until the age of 2 years old. Evidence: Child #9 - Immunizations on-file dated November 2018 when the child was 13 months old.

Plan of Correction: The vendor will ask the park to provide current immunization records.
