This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on April 26, 2021 and concluded on April 28, 2021. A complaint was received by the department regarding allegations in the areas of staffing and supervision and building maintenace. The provider was contacted by telephone to conduct the investigation. The inspector emailed the provider a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-270-A Complaint related: Yes Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence:
On the outside play areas, a Little Tikes playground play structure was broken with the top pieces of the play structure laying around the structure. A plastic water table was cracked which could snag skin. The large plastic play structure had a bridge in between the two platforms, the bridge had cracks in the plastic which could snag skin. The fence around the play area was leaning and warping.Plan of Correction: Inspection was done via facetime/duo
virtual call. This method does not
accurately describe the said evidence.
The Little Tykes tree, simply snaps
together. This item is not broken.
The plastic water table, that had cracks
was disposed immediately after the
inspection.
The large structure will need to be
disassembled and removed as replacement
parts are not available from Step 2. This
will be done promptly.
The fence is composite, and is not warped.
It is a matter of cosmetic appeal, and does
not affect the integrity or safety of the fence. The fence
is leaning, because of the abundance of pea gravel
(resilient surfacing) that is pushing against the posts.
Per the standards, fencing is not required.
We will remove all damaged items, before the
end of the week. Until this is complete, children will not
use the broken play structure.
