Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 4/30/2021. The director was contacted by telephone to initiate the inspection. There were 36 children present in the Licensed programs and 8 staff. The Inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and one violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, it was determined that one record does not include all documentation of a physical exam as required.
Evidence:
Child record A does not include documentation of a completed physical examination by or under the direction of a physician. This child has been enrolled more than 30 days.

Plan of Correction: I will ask the parent for a full physical, signed by the doctor.
