This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on April 28, 2021 and concluded on May 5 2021. A self-reported incident was received by the department regarding allegations in the areas of following the licensees own policies and procedures. The director was contacted by video call to conduct the investigation. The inspector contacted the director with a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued.
Standard #: 22VAC40-185-40-E Description: Based on review of the center's policies and procedures, staff interviews and video footage, it was determined that the licensee did not ensure that the center?s own policies and procedures that are required by the standards were maintained.
The center's policies and procedures regarding supervision of children states that staff must "count the children using attendance tracker in the iPad before leaving the classroom and playground".
Procedures also require when "coming back inside/transition: 1. Line up children, teacher will do a name to face using the iPad tracker. 2. Before leaving the playground, the teachers will sweep the whole playground looking in any area that children can hide. 3. Through every new doorway staff will do a name to face. 4. When staff reaches their destination staff will cross reference with the roll call sheet".
On April 27, 2021 between approximatley11:20am and 11:30am staff #1 is observed allowing the children to enter into the classroom one at a time without following the centers policy of conducting the name to face check.
Staff #1 did not follow center procedures and complete a name to face check using the iPad tracker.
Staff #1 did not conduct a sweep of the outside fenced area to check areas where children can hide.
Plan of Correction: Policy is that the teacher must not only do a swoop of any location they are leaving, but also look around and behind any hidden option clearing openly to other teacher.
