Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/5/21 and concluded on 5/7/21. The director was contacted by telephone to initiate the inspection. There were 24 children present with 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-550-M
Description: Based on record review and interview, the center failed to maintain a written record of a child's injury that included all the required components.

Evidence:

1. The injury report dated 10/16/20 only contained one signature and did not document the method of notification.
2. The director stated the parent was notified at pickup and she is unaware of why the report was not document and why the parent was not asked to sign the report.

Plan of Correction: A staff training was conducted last week about mandated reporting and incident reports to give everyone a refresher.
