A complaint investigation regarding classroom staff:child ratios, staff background checks, and staff qualifications was conducted today. This investigation was conducted using an alternative inspection protocol in response to the current health emergency declared by the Governor of Virginia.
Based on the information gathered through staff interviews and a review of administrative documents, this complaint was deemed valid.
Ongoing compliance with the Minimum Standards is the responsibility of the licensee.

Standard #: 22VAC40-185-350-E-1 Complaint related: No Description: Based on management staff interviews and a review of classroom attendance records, it was determined that the required staff:child ratio for children from birth to the age of 16 months: one staff member for every four children was not followed.
Evidence:
On 5/6/2021 from 9:30-4:00 the Infant B classroom was operated beyond the allowable staff:child ratio of 1:4. During this time there were 5 children in care with only 1 staff person.Plan of Correction: Additional staff have been hired. Moving forward all classrooms will operate within required staff:child ratios at all times.
Standard #: 22VAC40-185-350-E-4 Complaint related: No Description: Based on management staff interviews and a review of classroom attendance records, it was determined that the required staff:child ratio for children from three years to the age of eligibility to attend public school one staff member for every 10 children was not followed.
Evidence:
On 5/6/2021 from 9:30-10:45 the 3's classroom was operated beyond the allowable staff:child ratio of 1:10. During this time there were 12 children in care with only 1 staff person.Plan of Correction: Additional staff have been hired. Moving forward all classrooms will operate within required staff:child ratios at all times.
Standard #: 22VAC40-191-60-B Complaint related: No Description: Based on management staff interviews and a review of staff background checks it was determined that one staff person was employed before the child day center received her completed sworn statement or affirmation.
Evidence:
Staff person B began employment on 4/15/2021. On the date of this investigation there was no Sworn Disclosure Statement completed in her record.Plan of Correction: The staff person did sign the form when hired but the current management team is not aware of where the previous management team placed the signed form. The staff person signed another Sworn Statement immediately during this investigation.
