Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on May 6, 2021 and concluded on May 7, 2021. The director was contacted by telephone to initiate the inspection. There were 12 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on staff interviews and a review of center documentation, the licensee did not ensure the operational responsibilities were followed which include, but are not limited to, ensuring the center's activities, services, and facilities are maintained in compliance with the standards of licensed child day centers, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Evidence: During the inspection on May 6, 2021, center staff confirmed that five children (child #1, #2, #3, #4 and #5) left the program about 3:35 pm with an adult who was not listed on their parent's authorization for pick up. The center's "sign in/sign out" policy is "the child will only be allowed to leave the program with someone other than the parent/legal guardian if advanced written notification from parent /legal guardian is provided". There was not written notification the adult was allowed to pick up the children from the program and therefor the center did not follow it's own policy regarding the sign in/out policy and the names of persons authorized to pick up the child.

Plan of Correction: Staff were reminded that parents need to submit a note stating that school staff can sign their child out. School staff will aloes be required to be added to the child's pick up list. Children will not be permitted to leave sign & sound of staff with out written documentation.

Standard #: 22VAC40-185-340-G
Description: Based on staff interviews and a review of center documentation, the licensee did not ensure children 10 years of age and older shall be within actual sight and sound supervision of staff except when the following requirements are met:

Evidence: During the inspection on May 6, 2021, center staff confirmed that five children (child #1, # 2, #3, #4 and #5) had left the program about 3:35 pm with an adult who was not listed on their parent's authorization for pick up. The children were out of sight and sound supervision of the center staff. Center staff indicated this happens for about 10-15 minutes a couple times a week and the children are helping an adult, who is not a center employee clean equipment.

Plan of Correction: Staff were reminded that parents need to submit a note stating that school staff can sign their child out. School staff will aloes be required to be added to the child's pick up list. Children will not be permitted to leave sign & sound of staff with out written documentation.
