Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on May 10, 2021 and concluded on May 11, 2021. The director was contacted by telephone to initiate the inspection. There were 45 children present and 16 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of medications, the center failed to ensure a written allergy plan for each child with a diagnosed food allergy was completed and on file.

Evidence:
1. Child 1M had a emergency medication due to a food allergy. There was not a written allergy plan on file for Child 1M.
2. Staff verified that there was not a written allergy plan on file that they only had the signed medication authorization paperwork on file.

Plan of Correction: Administration contacted the parents and requested if the medication was to come back to the center that they would also need to have an allergy medication form on file. There have been additional staff members trained in Medication Administration. Moving forward administration will put in place a process where the medication will be reviewed by a MAT trained staff member and followed up by administration to ensure all required components are on file and completed.
