Technical Assistance:
1. Discussed the reasons personal products are to be stored in an area out of reach of children.
2. Reviewed/trained vendor on new subsidy requirements.
3. Forms provided: attendance, new emergency plan, and proof of identity.

Comments:
10:25-11:45am Annual unannounced subsidy inspection conducted. Observed 2 children = 6 points with the vendor. Ratios and supervision were in compliance. Children were playing with toys, singing songs in Spanish, and being held. The vendor was talking and playing with the children. The daycare areas of the home were found to be clean, safeguarded appropriately, and well supplied with toys and equipment for the children. The vendor is currently certified in First Aid/CPR. Safety non-compliances found.

Violations:
Standard #: 22VAC4-665-340-H-1
Description: Based on observations made, it was determined that a crib in use today does not meet the 2013 federal safety standards. Evidence: According to the manufacturer label on the brown portable crib it was made in 2009, and therefore does not meet current safety standards. This crib was being used by Child #5 today.

Plan of Correction: The vendor will get a new crib made after 5/1/13.

Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 1 of 2 children's records did not include required information. Evidence: Child #4 - There was not a name, address + phone number for an emergency contact person. The parents were listed.

Plan of Correction: The vendor said she will ask for information on a third person she can contact in an emergency. This information will be written on the form where it is highlighted in orange.

Standard #: 22VAC40-665-240-C-2
Description: Based on observations made, it was determined that electrical outlets did not have protective covers. Evidence: Two outlets in the main play area did not have covers today.

Plan of Correction: The vendor said she will cover them today.

Standard #: 22VAC40-665-370-E-3
Description: Based on interview conducted, it was determined that the diaper changing pad has not been cleaned with soap + water and sanitized with bleach + water after each use. Evidence: The vendor said she has been cleaning it with soap + water.

Plan of Correction: The vendor will make a bottle of bleach + water to spray on the diaper pad after it is cleaned with soap + water.
