Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on May 20, 2021 and concluded on May 21, 2021. The director was contacted by telephone to initiate the inspection. There were two children present and three staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility

Violations:
Standard #: 22VAC40-665-580-B
Description: Based on review of two staff records and interview, the provider did not ensure the two staff completed the required VA Preservice Training for Child Care Staff within 90 days of employment.

Evidence:

1. The record for Staff #1 (hire date 06/03/2019) did not contain documentation of completing the Virginia Preservice Training.
2. The record for Staff #2 (hire date 09/01/2020) did not contain documentation of completing the Virginia Preservice Training.
3. Administrator confirmed that Staff #1 and Staff #2 did not have documentation of completing the Virginia Preservice Training in their records.

Plan of Correction: I will make sure that the staff all complete the VA Preservice Training and put the documentation in their records.
