22VAC40-185 STAFFING AND SUPERVISION.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/8/2021 when a self-reported incident was received by the department regarding allegations in a the area of supervision. The director and was contacted by phone to conduct the investigation. The inspector communicated with the director and other staff to complete the investigation. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and a violation was issued.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interviews, the center did not maintain sight and sound supervision for children under 10 years of age.
Evidence:
1. The inspector interviewed 2 staff who verified that child #1 was left alone out of sight and sound supervision for 1-3 minutes inside by the bathroom. Staff #1 stated she found child #1 by the bathroom. Staff #2 stated she left the child in the bathroom and returned back outside with the other children. The approximate time child was left alone was 1-3 minutes.

Plan of Correction: Staff was retrained on center policies.
