Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 05/10/21 and concluded on 05/20/21. The director was contacted by telephone to initiate the inspection. There were 79 children present and 19 staff. A licensing inspection was conducted on the
same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on a review of records and interview, the vendor did not ensure that four out of five staff submitted documentation of a negative tuberculosis (TB) screening within the required time frame.

Evidence: 1) The record for Staff #2, hired on 04/26/21, did not contain documentation of a negative TB screening. 2) The record for Staff #3, hired on 02/22/21, contained the results of a negative TB screening that were dated 11/25/19. 3) The record for Staff #4, hired on 03/04/21, contained the results of a negative TB screening that were dated 06/12/20. 4) The record for Staff #5, hired on 05/03/21, contained the results of a negative TB screening that were dated 08/20/20. Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Per the Vendor: "Staff #2 now has current test on file. The other 3 staff have made apts to get a current test with a deadline of 5/31/21. Going forward [Assistant Director] will check TB test to make sure they are no older than 30 days."

Standard #: 22VAC40-665-780-A-3
Description: Based on a review of documents and interview, the vendor did not ensure that lockdown procedures were practiced at least annually.

Evidence: 1) The licensing inspector reviewed the emergency drill log for the year 2020. A lockdown drill was not documented. 2) During interview, a member of management reported the required lockdown drill was not practiced annually in 2020.

Plan of Correction: Per the Vendor: "Lockdown drill scheduled for June. Going forward [Staff] will be scheduling and conducting monthly drills."
