A monitoring inspection was initiated on Tuesday, May 18, 2021 and concluded on Thursday, May 20, 2021. The director was contacted by telephone to initiate the inspection. There were 57 children and 4 staff present for this inspection.
The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-140-A Description: Based on review of children's record and interview, the center did not ensure to obtain physical within one month after attendance.
Evidence: In Child#4's record (start date: 09/08/20), there was no physical documentation found. Interview with staff confirmed there was no available physical documentation for review during this inspection.
Plan of Correction: Parent has been contacted and updated physical has been scheduled. Student physical will be turned in on Tuesday, June 1, 2021.
