63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 5/13/2021 and concluded on 5/14/2021. The director was contacted by telephone to initiate the inspection. There were 71 children present and 18 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not implement monthly practice evacuation drills for each month.

Evidence:
1. A monthly evacuation drill was not documented for the month of December 2020.

Plan of Correction: I will schedule them and put them on a calendar to make sure they are completed.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 1 record of injury did not contain all required information.

Evidence:
1. An injury report dated 5/11/2021 did not contain documentation of any future action to prevent recurrence of the injury.

Plan of Correction: I will return the form to staff if it is not complete.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review, the center did not obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.


Evidence:
1. Documentation in the record for Staff #1 (date of hire: 10/28/2019) indicated they have resided outside of the state of Virginia within the past 5 years.
2. The record for Staff #1 did not contain documentation of a sex offender registry search for the state of Maryland.
3. The record for Staff #1 did not contain documentation of the results of a search of the child abuse and neglect registry for the state of Maryland.

Plan of Correction: The center will provide documentation that the state of Maryland sex offender registry search and a child abuse and neglect registry for the state of Maryland for Staff #1 was requested within 10 business days.
