Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 05/25/2021 and concluded on 06/02/2021. The director was contacted by telephone to initiate the inspection. There were 52 children present and 12 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed four children?s records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-550-D
Description: Based on a review of documents and interview, the center did not implement a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Evidence: 1) The licensing inspector observed the center's emergency drill log for the year 2020. The most recent shelter-in-place drill was documented in August 2020. 2) During interview, a member of management confirmed a second shelter-in-place drill was not practiced in 2020.

Plan of Correction: Per the Center: "Corrected immediately. Reminders added to Google Calendar through the Four Seasons Daycare email to be received 3 days before scheduled practice Shelter in Place drills. Currently one has been completed for 2021 and the next one is scheduled for October 2021."

Standard #: 22VAC40-185-550-M
Description: Based on a review of five injury records, the center did not ensure one of the written records of children's serious and minor injuries contained the required information.

Evidence: 1) One injury record reviewed did not contain documentation of the date, time, and how parent was notified.

Plan of Correction: Per the Center: "Corrected immediately. Accident report now has a space for parents to record the time notified when they pick up their child. See attached accident report."

Standard #: 63.2(17)-1720.1-B-4
Description: Based on a review of four staff records and interview, the center did not obtain a copy of the results of a criminal history record check and a sex offender registry check within the required timeframe from any state in which one individual had resided in the preceding five years.

Evidence: 1) The record for Staff #3, hired on 05/10/21, indicated the staff had resided in two other states outside of Virginia within the last five years. The record did not contain a sex offender registry check from one of the states where the staff member had resided. The record contained a sex offender registry check from another state, but it was obtained on 05/26/21. The out-of-state sex offender registry is required to be obtained prior to hiring. The record did not contain the results of a criminal history record from one of the states the staff member had resided. An out-of-state criminal history record check is required to be obtained prior to hiring. 3) During interview, a member of management confirmed this information during the remote inspection.

Plan of Correction: Per the Center: "Corrected immediately. Moving forward Directors will use the check list attached to ensure all pre-hire requirements are complete. New hires will not be able to star until all items are on file."
