Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 06/01/2021 and concluded on 06/09/2021. The director was contacted by telephone to initiate the inspection. There were 61 children present and 10 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1) The most recent immunization record for Child #1 (dob (10/14/2017) was dated 07/13/2018.

Plan of Correction: Will request parents to provide this information.

Standard #: 22VAC40-185-140-A
Description: Based on record review, it was determined that the center did not request a physical examination by or under the direction of a physician for each child within one month after attendance.
Evidence:
1) The record for Child #2 (start date 04/02/2020) did not contain a physical examination available for review.

Plan of Correction: Parents have the form. They will submit the information after their appointment.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review and interview, it was determined that the center did not request at least annually parent confirmation that the required information in the child's record is up to date.
Evidence:
1) The most recent documentation of child updates and confirmation of up-to-date information in Child #1's record was dated 06/12/2018.

Plan of Correction: Will request that parents review this information.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, it was determined that the center did not ensure that Central Registry search results are obtained for all employees within 30 days of employment or volunteer service.
Evidence:
1) The record for Staff #1 (start date 08/31/2020) did not contain Central Registry search results available for review.
2) The director stated that a Central Registry search request was sent on 08/14/2020 but no follow up was conducted.

Plan of Correction: We will submit a new Central Registry search request for Staff #1.
