Technical Assistance:
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated and concluded on June 16, 2021. The director was contacted by telephone to initiate the inspection. There were 46 children and 8 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center failed to obtain documentation of a physical examination by the 30th day of attendance.

Evidence:
Child #1's records do not have documentation of a physical by the 30th day of attendance.

Plan of Correction: The center will ensure that a newly enrolled children will have documentation of a physical by the 30th day of their attendance.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the center failed to obtain documentation of immunization on the first day of attendance.

Evidence:
Child #1's record did not have dated documentation of the child's immunizations on the first day of attendance.

Plan of Correction: We will assure that all new children will have documentation of immunizations on the first day of attendance.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training documentation, it was determined that two staff have not completed all required components for orientation training.

Evidence:
1. Staff #3 does not have documentation of orientation by the end of the first day of assuming job responsibilities. Date of hire 8/21/2017 -date of orientation 9/22/2017.
2. Staff # 1, #2, #3 and #4's records do not contain documentation of Child Abuse and Neglect training.

Plan of Correction: 1. We will ensure that all new hires, will receive orientation by the end of their first day of assuming job responsibility and will have documentation of that orientation.
2. We will ensure that all staff have "Child Abuse and Neglect Training" by June 21st, 2021.

Standard #: 22VAC40-191-60-B
Description: Based on a review of staff records, one of the staff records did not contain a completed sworn statement dated on the first day of employment.

Evidence:
Staff #4's record did not contain documentation of a Sworn Statement or affirmation dated for the first day of hire. Date of hire was 10/1/2018 and the sworn statement was dated 10/22/18

Plan of Correction: We will work to ensure that all new hires will sign the Sworn Statement or affirmation on the first day of assuming job responsibilities.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff records, one of the staff members does not have a central registry finding within 30 days of employment.

Evidence:
Staff #2's record does not have documentation of a central registry finding within 30 days of employment. Date of employment 11/6/202 - date of central registry finding was 6/8/2021.

Plan of Correction: The center will ensure that all new hires will have documentation of a central registry finding within 30 days of employment.
