This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on June 7, 2021 and concluded on June 7, 2021. The director was contacted by telephone to initiate the inspection. There were 31 children present and five staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-540-A Description: Based on review of three staff records and interview, the vendor did not obtain documentation of a negative tuberculosis (TB) screening within the last 30 calendar days of prior to beginning employment for two staff members.
Evidence:
1. The record for Staff #2 (Hire date 04/05/2021) contained documentation of a negative TB screening dated 04/29/2021.
2. The record for Staff #3 (Hire date 04/05/2021) contained documentation of a negative TB screening dated 04/19/2021.
3. A member of management stated she did not realize this was a subsidy requirement during the virtual inspection.Plan of Correction: All staff hired in the future will be required to submit a copy of their TB test prior to beginning employment.
